Snow Ball Permission Slips

Lionsgate Academy’s Snow Ball will be Friday January 13 at the LGA gymnasium. Students must fill out the Snow Ball Permission Slip in order to attend. Students may also bring one guest. In order for the guest to be allowed into the building, a guest permission slip form must be filled out and turned in. All forms must be submitted to Ms. Mills in room 18 by January 5.  If a student doesn’t have a signed permission slip on file and shows up to the dance, his/her parent/guardian will be called to pick up the student.
